Overview

Raktamokshana is the Ayurvedic procedure of therapeutic bloodletting – the controlled removal of a small quantity of blood from a region to relieve conditions driven by vitiated blood (rakta) and pitta. It is counted among the five main purificatory procedures (Panchakarma) and is always a doctor-performed, hygienic procedure.

At ACTYMED it is offered selectively for specific indications such as certain localised skin conditions, gout (vatarakta) and localised venous congestion, using methods such as medicinal leech therapy (jalaukavacharana) where appropriate. It is done with informed consent and careful patient selection, not as a routine treatment.

How is it performed?

Preparation (purva karma): suitability is assessed, including whether the patient is fit for blood removal, and the site is prepared. Main procedure (pradhana karma): a small, controlled quantity of blood is removed by the chosen method – for example a medicinal leech, or a controlled superficial technique – under hygienic conditions. Aftercare (paschat karma): the site is dressed and cared for, rest and a light diet are advised, and healing is monitored.

Which conditions is it used for?

Localised skin conditions such as some eczema and psoriasis patches, gout and vatarakta, localised inflammatory swellings, and venous congestion. It is targeted, not general.

What are the classifications?

Broadly two – shastra methods using instruments (siravyadha or venesection, and pracchana or controlled scraping) and anushastra methods using accessories (jalauka or leech, alabu, and cupping-type suction).

Who should avoid it?

Anaemia, generalised swelling, bleeding disorders, pregnancy, significant debility, and very young or frail elderly patients. Careful screening is essential.

Is there any modern evidence?

Medicinal leech therapy has some clinical support – for example in knee osteoarthritis pain and in relieving venous congestion in reconstructive surgery – while other forms have limited formal evidence. We present it honestly and use it only where indicated.
