When does a sports injury need surgery?

A minority do: complete ligament ruptures in unstable joints (some ACL injuries in pivoting athletes), displaced fractures, full tendon ruptures, and locked knees from displaced meniscus tears. Most sports injuries — including many complete tears — rehabilitate successfully without an operation.
